when I was a medic we once called a guy who was in persistent vfib for 45 min.....

MVA trauma code, transported rapidly from scene, lots of fluids, multiple rounds acls meds(we gave bretylium and high dose epi back then....). arrived at hospital in vfib and remained in same for 15 more min at which point er doc called code. he was defibed > 30 times I think. chest hair caught on fire at one point. not pretty. pt ended up having a traumatic aortic dissection from his steering wheel.

I manage a small cohort of elderly patients in my local hospital as there PCP. When one passes I am the one that does the death certificate and items such as that. The state even has the entire thing done online now. I am sure it will vary from state to state though, so you just need to find out.
