I don't want my home address floating around esp when narc patients have a script with my dea # on it that they can look up in the interweb...just sayin' Re: the per diem, I think u can only have one DEA regardless of where you work. Depending on state, I would assume u would just need a new set of triplicates with yours and your SPs info on it.
