Dealing with Attestations and annual Medicare physicals

No comments yet?  Is anyone dealing with WellMed or Optum Medicare supplements that require you as the person who saw the patient to address each and every diagnosis with DETAIED explanations of status (active or inactive), treatment, outcome, EVEN if seen by a specialist who determined the diagnoses!!!  I believe some practices are all in with this  (unfortunately) and others do not follow what these 2 supplements want. The first is extremely time consuming.  Is the reimbursement enhanced to the practice that follows what these companies want?  Any input would be appreciated.  I think this directed at primary care only.
