TWR
66 y/o IDDM (controlled) and lymphoma in remission has experienced muscle cramping anywhere in the body but usually in lower extremities. These began long before the IDDM and Lymphoma. Labs all normal but Mg is on the low side. Has taken Quinine and Mg supplements without success. He is not a big water drinker but does drink 64 oz. of coffee daily. Any thoughts.?
GetMeOuttaThisMess
Have you checked for muscle weakness (proximally in LEs and distally in UEs), and from a lab perspective, a CK with iso-enzymes to exclude myositis? Is pt. on a statin with the DM?

UGoLong
Low mag can do it. What was the level? Max oral mag supplementation without side effects is usually 400 mg bid. See if you can get mag up to 2.0 and see how the patient feels. If tablets won't do it, you can try milk of magnesia 5 ml bid. If he tends to diarrhea (with all that coffee!), that can lead to low mag, as can PPI use. Yeah, it could be lots of other things, but I'd fix his lytes (K, Mg, and Ca) first.
