* How long have you known the applicant?

* What have you seen him or her accomplish?

* Based on your personal experiences, how do you compare this applicant to others seeking a similar program or position (or those in the target profession you know)

A pearl taught to me by a old friend...I only write one letter of rec a year so I say that in the letter. Something along the lines of "I am often asked to write letters of recommendation and I only write one per year. This year I am recommending *candidate*."
