Moderator EMEDPA Posted December 31, 2021 Moderator Share Posted December 31, 2021 I had a pt today with a hip fx not responsive to 100 mcg of fentanyl and 3 mg of dilaudid. I asked CRNA to do a femoral nerve block and 15 min later the pain is mostly gone. I watched him do it. Does not look terribly difficult. He said he would watch me do the next one and sign me off for see one, do one :).
Moderator LT_Oneal_PAC Posted December 31, 2021 Moderator Share Posted December 31, 2021 Do them all the time. It’s great! No more snowing little old ladies trying to control their pain, no repeat dosing. Just set it and forget it! 1
ohiovolffemtp
Never done one. Have done hematoma blocks for hip fx and had good success reducing but not eliminating pain. @EMEDPA: did they do it under U/S guidance?

Yes. using the linear probe, visualize the femoral artery and the nerve is a big white bundle just lateral to it. Infiltrate 20-30 cc of anesthetic around it, aspirating back every 5 cc to make sure you have not entered the artery.
