I googled my NPI number this morning and to my surprise there were multiple websites with my full name, personal address, personal number along with business address and number! You have to log in to the NPI database and see if you can change your mailing to your business address and your phone to the office phone. Guess I need to let the DEA know as well, though I didn't use it, or maybe just let it expire without renewal. I just checked mine, and all of the addresses and information shown is from the company that set up my NPI for me. It doesn't show any personal information like you describe.
