LOR Etiquette

So, here's my situation. I have two out of three LORs who have finished and submitted their letters. I'm looking to submit early (like mid-May). I have one LOR who has not started or completed his letter, has not responded to my initial email (with instructions and a brief bio sort of thing to aid in writing the letter), and has not responded to a gentle follow-up email (asking him simply to verify that he received the CASPA email).

 

My options:

 

1) Submit with or without him and hope he gets it in before the application deadline in November (lol).

 

2) Remove him and use another LOR writer (I have another equally good if not better one, although his reliability is also somewhat suspect).

 

What do you guys think?
