I'm 39, so I listed stuff as old as twelve years at least. But, older would even be fine in my opinion. It shows your track record as charitable and not just recent involvement.

My guidance would be to err on the side of including things that 1) Showed a multi-year commitment to something.  Volunteered in the special olympics once when you were twelve, and never since? Ooookay, why does that make me want you in my program? or 2) Are specifically healthcare related.  So, yes, medical missions trips as a teen with your church group, even if you weren't certified and were just a gofer is a good thing to keep including.
