Genital warts on a baby

Hey guys saw a an approximately 1.5 year old female patient in the ER a few days ago whose mom said had a rash for "one week". She asked could it be her eczema or an allergic reaction to her diaper..... um....she had warts all around her rectum. Anyone dealt with this? My attending saw and agreed with diagnosis. I called Pediatrician who wants DFACS contacted (obviously) and for the patient to be seen the next day for urgent dermatology referral. Up To Date said its not always sexual abuse but man...I called Pediatrician's office today who said they showed up and they also agreed with dx. Like WTF. I know it happens but seriously.  Mom seemed concerned but denied knowing of anyone that could have abused her and denied personal history of genital warts that may have been transferred at birth. Red flag - those warts have been there longer than a week (maybe like 30 around the rectum and spreading toward vagina. I also saw that baby had been drug tested at birth so there must have been some concern about the mother from the beginning (it was negative0. Dad showed up about an hour later after she told him what we were concerned about. He denied knowing of anyone around that could have been abusing her but says he's at work during the day so he couldn't be completely sure. She doesn't go to daycare. I feel like if I were her parent I would be hysterical and brought her in long before then.

 

I have a baby now so maybe it just hits me harder now. Just venting.

Hang in there, man.  We aren't equipped to fight all the evil in the world, just recognize and correct that little bit that walks through our doors.  You did the exam right, involved the right people, the boat is well and truly loaded with what sounds like several righteously PO'ed specialists, and the authorities are notified.

 

The big trick is to congratulate yourself for doing the right thing right, and not beat yourself up for all the stuff you can't do because it's not your place or your job.  You made the personal call to make sure the pediatrician was up to speed, and that's about all you can do.

Sounds like you handled it perfectly, and ours is not to judge, that is the legal system and DSS/DCF or what every your state agency is.... our responsibility it to recognize and report!

 

Doesn't make it any easier though.....  sorry you had to live it, no fun...

UGH is right! I used to work boys that were sexually abused at an early age. I don't know if you heard about the Jared Fogel case? Jared Scot Fogle aka, as the Subway Guy,

On August 19, 2015, he agreed to plead guilty in federal court to possessing child pornography and traveling to pay for sex with minors. A hearing was scheduled for November, during which Fogle is expected to formally enter his plea and be sentenced. The content is consistent,” the affidavit said, “with the purpose of the original investigation of bestiality and with a sexual interest and attraction to children.” Source: indystar.com It takes all kinds. You never know what goes behind closed doors. I must admit, when I first heard this  I thought there has to be a mistake, he doesn't fit the typical pedophile profile
