Surgical 1099 Independent contractor

I'm starting the process of working as a 1099 independent contractor, strictly as a 1st assist in Orthopedic surgery. If anyone has experience with this topic, how do you have reimbursement set up for your services? do I set up an agreement through a Medical Billing company to bill insurance for my fees assisting in surgery, do you set up a contract to bill the hospital at say an hourly rate/flat fee, or do you bill the surgeon an hourly rate/flat fee? 

Also, is there any difference when billing for your services through a hospital vs a surgery center?
