I know I have to get a new delegation agreement with my new supervising physician but do I have to apply for another CDS license? What about my DEA license? Febrifuge Posted May 23, 2015 Share Posted May 23, 2015 In Minnesota, where it sounds like things are at least a little similar, we file a new Delegation Agreement (formerly supervision agreement) with any new (or additional) supervising MD. We file a new Notice of Intent to Practice with any new job site or employer. But the state license is ours and independent of any of that. DEA is a federal agency, and a federal certification, so that is definitely yours, but on the other hand it turns out that each DEA number is linked to a provider AND a practice site.
