Basically, a guarantee to the adcom that the applicant will attend the program if offered a seat. A few of my friends did the same when they were waitlisted by their top choice programs. Some of them ended up getting offered spots, while some didn't. Not really sure how much the LOI's helped in each case. Definitely can't hurt your chances though.
