If a program wants documentation, they should let you know on their website. Simply keep your own notes on whom you shadowed when, and you'll be good to go in the vast majority of cases. If any program actually requires verification/proof of shadowing hours, from someone who actually has any real patient contact hours, run away. :-) Link to comment Share on other sites More sharing options...
flyingsquirrel Posted December 3, 2012 Share Posted December 3, 2012 The MOST you should be asked to provide is the name and contact info of the person you shaddowed...just like you would provide the contact info for your former supervisors when applying for a job.
