Danielledeperrot Posted June 9, 2020 Share Posted June 9, 2020 I have started requested evaluations on CASPA and I have just been putting the evaluators first and last name. Should I be included their title (MD, PhD, PA-C)? Or is there somewhere that they will specify that later on?
