Shadow a PA or MD/DO?

Both.

 

Shadow someone in primary care, someone in the hospital, someone in a specialty. Shadow a doc who has PAs in the practice, shadow a PA who flies solo and has a lot of autonomy, shadow an NP.

 

Do that, and you'll have an advantage over most of the people you are applying with, and most of the people talking in the pre-PA sections of the boards.
