Advice Please - Derm PA Employment Offers (FT/PT)

Good afternoon everyone,

I have been offered a part-time Derm PA position in Palm Beach (my hometown), Florida. I am wondering what I should expect to see in a proposal for a part-time PA position. Is it typical to also be offered benefits (medical, CME's, malpractice insurance, etc.) being a part-time PA?

I also have one other fulltime Derm PA offer (relocation would be necessary-rent averages $600/month) working 10-12 hour days with a salary of $76K for first 6 months(the previous PA-C verified I would be working these 10-12 hours for the first 6-12 months due to paperwork, etc.). Then salary raise to $82K after the 6 month review. They also offered:

CME: $1K

Holiday: 6 days

Sick: 1 day

Malpractice: $1200, plus listed as additional physician's policy.

Bonus plan: To be negotiated after 1 year.

Vacation: 2 weeks

I will be speaking to the physician offering me the PT position this week to get the details of the proposal. Any advice/thoughts are appreciated!! Thanks!

Good afternoon everyone,

I HAD been offered a part-time Derm PA position in Palm Beach (my hometown), Florida over the phone. After receiving the proposal via email, the schedule was more of a fulltime week (38 hours/week).

Salary: $65K

Bonus: "PA will be eligible to receive 2%, 5%, 7.5%, or 10% of PA's excess net receipts in each calender quarter after PA generates, in receipts, 4x, 5x, 6x, or 7x, respectively, of PA's quarter base salary. As receipts equal the fourth bonus tier, PA shall receive a Total Cumulative Quarterly Bonus in one year of $10,287.50." If you would not mind looking at the plan completely with examples and letting me know what you think about it, please allow me to email it to you at your request. I am not sure if this is a good plan to accept.

CME: $1,500K.

Holiday: 8 days

Sick: 5 day

Malpractice: Listed as additional physician's policy.

Simple IRA Plan: Match dollar for dollar up to 3% annual income.

Disability coverage: Paid by practice

Health insurance: 50% paid by practice

Vacation: 1 week

Then salary raises to $82K and schedule will be a 4.5 day week after the 6 month review. They also offered:

Vacation: 2 weeks

I choose #1, the base pay is deceiving b/c of the chance for bonuses. IOW, if you work your butt off you will have something to show for it. Benefits are better and it's right in your hometown. But the question is "What do you feel is the right fit for you?"....your going to work hard no matter where you work, it's just nice to have something to show for it.

If you are interested in dermatology you really should join the sdpa. Go to Dermpa.org. This is the best site for fellow PA's who want to work or do work in Dermatology.

From what you stated above I believe you are going to be underpaid. There are certain things you need to know as a Derm PA.

1) Are there any restrictions on what type of pateints you can actually see.

2) Will you get a copy of your collections each month to review.

3) What patient load are you expected to have.

4) Will you have a Medical Assistant that works with you only or do you share with the other providers or maybe you don't have one at all.

I can tell you the best way to be paid in Dermatology is to be paid on a percentage of your collections. It should be somewhere between 30%-40% of what you bring in.

Example: your monthly net collection is $40,000 a month which is easily attainable seeing 25-28 pts per day 5 days a week.

You get 30% your pay would be $12,000 per month. The practice makes $28,000 a month. A win win for both.

If you are only paid a salary $82,000 per year and the same numbers are used as above you would be making only $6,833 per month or about 17% of your collections, and the practice makes 83% or $33,200 per month. Not a good deal for you but a great deal for the doctor.

I would recommend becomming a member of the sdpa and get informed about how PA's get paid in derm, because a lot of PA's are being severly underpaid for the work they do.

I don't have much time for an in-depth post, but the second full time offer struck me as borderline ridiculous. 10-12 hour days (in derm?!) for $76-82k? That seems excessively low to me. I also advocate joining SDPA to get more relevant salary information. There are a multitude of ways to structure your contract so that it is acceptable to your employer as well as you. I just noticed your original post was over a month ago. Any updates?
