How do you calculate pediatric dosing?

I am looking for help on calculating pediatric dosing with epocrates. For example,

 

Amoxicillin 80 mg/kg/day PO div q 12h x 5-7 days

 

Dose: 80 mg/kg/day

Weight: 29 kg

Freq: BID

From: Liquid

Strength: 400/4mL

 

Per Dose = 14.5 mL

 

I understand there is 5 mL per teaspoon, so does this mean 3 teaspoons BID? If that number were to equal 7.5 mL would it be one teaspoon and a half? Any suggestions would be great, thanks.

Then I calculate on Pepid and get a completely different answer:

 

Amoxicillin

 

Standard Dose: 40 - 45 mg/kg PO q12hr

 

Selected Dose: mg/kg

 

Patient Weight: 29 kg

 

Confirm Concentration:

 

 

Unit Dose: 875 mg PO q12hr

 

Caution!!

* - Indicates maximum recommended dose.

 

Calculated Dose Range: 875 - *875 mg

 

Amount to give: 10.94 mL

So - first of all, do yourself a favor and pickup the tarascon pharmacopeia. It has a pediatric dose scale in the front of the book that will make your life WAY easier with this. The calculations in there run around 50mg/kg/day; 100mg/kg/day for OM.
