What do you use for acute pain management for patients taking buprenorphine

Recommended Posts

Had a patient with severe RUQ abdominal pain who takes scheduled buprenorphine.  He was "large", approx 175 kg and 5'4" I tried dilaudid hoping that would overcome the buprenorphine - minimal success.  I was reluctant to use toradol as it was possible he was surgical.  IV acetaminophen (Ofirmev) helped some.  IV lidocaine - no effect.  Subdisassociative ketamine worked briefly but patient hated it because it made him see dead relatives.  I wound up calling anesthesia, who said that toradol would be OK, even if the patient was a surgical case.  The on-call surgeon happened to wander through the ED and reluctantly agreed.

Ultimately the patient wasn't surgical and his pain diminished.  However, I'm wondering if there are other drugs that would have been useful.  Does anyone have anything they've used with success?  Stadol?
