What is the best way to go about setting up a practice agreement/contract? Is there a boilerplate legal form we could use? Should we use an attorney? Also, has anyone been to Charmaine Baker-Nelson's housecall course in New Orleans. It looks pretty useful but the price is kinda high. Thanks.
