credentialing in MD

I have some questions for those who are practicing in private practices in Maryland. I believe you all have been in the process of getting credentialed by insurances. Are you legally allowed to see commercially insured patients without a supervising physician on site? Because my practice has been told that many insurances (Carefirst, Cigna, Aetna for example) don't recognize PAs, so in order to bill " incident to", I have to have a supervising physician on site. I just feel that is ridiculous because when my supervising physicians are away, I can't see any commercially insured patients. I have seen PAs in urgent cares seeing patients without a doctor on site.  So I am very confused as to what types of patients I am allowed to see, etc. I really appreciate your help if you can give me some insights. Thanks!
