Alright fellow PA's

 

 

This is important

 

Joint Commission is asking for comments on the medical home model. Some of this is about rather PA and NP ("Non-Physician Providers") should be allowed to lead the team - This is a HUGE issue in that the AMA is firmly against us.

 

We really need to take 10 minutes, read through the literature, take the survey and write in an extra comments or send in extra comments to the joint commission so we can be heard. It does not have to be a long email, should only focus on our abilities (no mud slinging or name calling) and prove to the joint commission that not only should we be allowed to CONTINUE leading the patient care (something every primary care provider has been doing already) but that we should be recognized and allowed to do so in the new regulations.

 

This is one of the few opportunities to be able to have our voice heard by a deciding body and we should take whole advantage of it.

 

PLEASE copy and paste this message to friends and co-workers that are PA or NP and have them make the effort to write in.

Field Review

In July 2011, a PCMH certification program was launched for accredited ambulatory care organizations. To date, 47 providers have been awarded PCMH certification, representing 437 sites of care. The revision of several of the PCMH requirements included in this certification ensure that they remain contemporary and keep pace with evolving medical home concepts and information from demonstration projects underway across the country and environmental changes, such as Stage 2 Meaningful Use.

 

The proposed revisions incorporate feedback from key stakeholders, revisions to clarify intent, and include concepts that align with Stage 2 Meaningful Use requirements.

 

The document listed below contains the proposed new and revised standards and elements of performance (EPs). The proposed revisions are presented in strikethrough and underlined bold text within a grey box. Please use it as a reference while completing the survey. Note: Prior to submitting your comments, download and print the document below. This document requires Adobe Reader.

The survey consists of 17 questions and should take about 10-15 minutes to complete. Please use the arrows at the bottom of each screen to move forward or backward within the survey.

Comments will be gathered for six weeks beginning March 11, 2013

and ending on April 22, 2013.

After reviewing the new proposal I can safely say they are getting better. It is critical that PAs and NPs be allowed to lead the medical home. We provide exceptional care and are an integral part of the primary care system in this country. As a primary care practice owner (see www.berkshiremobilemedicine.com) and a PA I am the primary provider for all my patients. I am 100% capable of doing this and highly respected in the community.

The thing that confounds me is the AMA's statements that PA/NP don't know enough to fill this roll. Show me ANY doctor out there that practices solo, in isolation and I will be the first to agree with them. The reality of the situation is that we are all part of a much larger team. We all ask questions, we all rely on specialists, we all function as part of a team. To say that a PA or NP can not be a leader of this team in a new developing model when we have already been leading the team since the 1970's just stinks of elitism and protectionism at the expense of patient care.

Please judge us on the QUALITY of care we deliver, not on the initials after our name.

I am on the AAPA Facebook feed and they have a PowerPoint slide displayed today with the heading of "what a pa is..." There are a few lines to it but smack dab in the center is a part that reads "...practice medicine as part of a physcian led team...."

 

Perhaps I am reading more into than I should but that seems to be counter intuitive to the point you are trying to convey.

 

Why is it so difficult to define our role in health care?

I took the survey and added comments at the end. There is one section of the requirements that specifically mentioned "licensed independent provider" section PC.04.01.03 EP3 and I believe the term "independent" needs to be removed. It will cause us problems if the word independent is left in. Please make comments regarding this section, too.

 

Regarding AAPA powerpoint slides: They also did not mention the 2000 hrs of education we have in our last year of training during our "mini-residencies". Slide number 9. I commented there also. Paula
