quarternote
Has anyone seen this question on the Internet? It may be old news. I came across it at http://www.testprepreview.com. 2. A 13 year old girl is admitted to the hospital with lower right abdominal discomfort. The P.A. should take which the following measures first? A: Administer Loritab to the patient for pain relief. (Not my spelling) B: Place the patient in right side lying position for pressure relief. C: Start a Central Line. D: Provide pain reduction techniques without administering medication. So...my answer would be "A" .....he says no that the correct answer would be "D" ...and I quote:... "2(D) Do not administer pain medication or start a central line without MD orders." Last time I checked I can write for schedule II. Since when do I need MD orders. Not to mention Lortab is spelled wrong. rcdavis
I guess I am missing The context of the question. None of the above would seem to be the right answer from an ED point of view. If, however, you are a hospitalist PA, then I guess pain relief would be an appropriate next step in the workup.
jdumoch
though I agree with rcdavis that none would seem correct, i think i would avoid giving PO pain meds as you must assume an appy... and i've seen anesthesiologists delay surgery because the patient was given PO... even if it is just one tab and a bit of water
quarternote
In Gen. Surg we routinely made people NPO post midnight except for Med's with a sip. I'm now in a hospitalist role. Guess that explains the difference. Regardless I would not wait for orders from an MD to decide on whether to give pain meds.

Here is the direct link: http://www.testprepreview.com/pance_practice.htm you are overthinking the question. D is correct. the first and quickest step would be a pain reduction technique such aselevating the knees and positioning for comfort. either way it is a stupid question. "Loritab is incorrect because it is PO and takes time to work, morphine would be more appropriate Right side lying position would INCREASE the pressure Central line placement is inappropriate
