Moderator EMEDPA Posted August 1, 2022 Moderator Share Posted August 1, 2022 I realized today that my primary place of employment was still using the 2005 Oregon form 827 workers comp form that states "signature of physician or nurse practitioner". This form was changed a long time ago to read "provider's signature", so this irked the hell out of me. . I printed out a copy of the current 2021 form, destroyed all the 2005 forms including the master, and replaced them with the current form.
