I am working on calculating my productivity. I work for a very busy total joints surgeon and I first assist as well as see patients in the office. I do not have access to my collections data. The only info I have for my calculations are what is seen on the medicare fee schedule look-up tool.  Where I am having trouble is estimating collections from private payers. I know they all vary, but I am just trying to get averages. Would anyone be willing to share an example of their collections report or at least give some kind of idea of how their medicare patients vs. private payer patient's collections compare.

Again, I am looking for first assist fees for total hips and knees as well as standard office visits and procedures (cortisone and visco injections). Thank you for your time!

It is always worrisome when you are working some kind of productivity and the practice won't open the books. I usually recommend that be a part of contract negotiations along with the right to an independent audit at your expense.

I think you touched on the problem of guesstimation. No matter what numbers you come up with your employer can say they aren't accurate because you don't have access to the actual numbers.

Absolutely...I have in writing that the numbers would be made available, and they aren't actually telling me no, they just "don't get around to it."

My solution to that is to make myself such a PIA they give me what I want. Go sit in the practice manager's office all day. Follow him/her to lunch and the bathroom. Text them before and after work. Make it important to them.
