Now I am leaving, the credentialing team told me they still will use it to collect insurance otherwise but will not use it for new credentialing/payor enrollment application. Should I get it revoke just for safety? Where do I find the revoke form. I ask and they did not offer it. Should I go to an authority or online form is sufficient?
