PA2O18 Posted January 4, 2021 Share Posted January 4, 2021 CDC now recommending 500mg ceftriaxone IM for pt <300 lbs and 1g if >300lbs. Doxy instead of 1g azithro if chlamydia is not ruled out. Moderator EMEDPA Posted January 4, 2021 Moderator Share Posted January 4, 2021 I have always used the one gram dose of IM ceftriaxone since someone in my training days mentioned that 1000 mg of ceftriaxone covers incubating syphilis and any lower dose does not. I like doxy > zithromax, but there is a compliance issue with bid dosing for a week vs single dose. Also zithromax covers a lot of other less common STIs, like mycoplasma genitalium(which is a common STI in Haiti) . Not sure if doxy does.
