Sleep Habits Among Full-Time College Students in Yakima County

Thank you for considering taking part in this survey measuring sleep habits among college students in Yakima County. The purpose of this survey is to fulfil the master’s research project requirements for the Heritage University Physician Assistant Program and to further our knowledge on the sleep habits and demographics among college students in Yakima County. This research is being conducted by Syvilla Reynolds, Thomas Loe, and Bassanio Martinez. The survey will take 20 minutes, but you can take as long as you need to complete the survey.

THIS SURVEY IS VOLUNTARY
Your participation in this survey is strictly voluntary. YOU DO NOT HAVE TO TAKE THIS SURVEY. However the information provided by you will help further our understanding of sleep habits among college students in Yakima County. At any time, during the survey, you may exit. You are not required to finish the survey.

AGE REQUIREMENT
At the time that you take this survey you MUST be at least 18 years old

SCHOOL REQUIREMENT
At the time of this survey you MUST be enrolled full-time (12 semester hours per semester [15 quarter credits per term] or more) at a college, university, or Trade School In the Yakima County, WA.

PRIVACY DISCLOSURE
This survey will be conducted anonymously. All data submitted via this survey will be collected anonymously. We will not be collecting or asking any information regarding email, address, name, or any other protected personal information. The survey will be administered via Google Forms. Google.com does not collect any of the information regarding email, address, or IP address. We chose to conduct this survey through Google Forms to maintain anonymity for the survey taker. No one will be able to identify you as a survey participant. All data will be maintained and secured In a password protected electronic format.

DATA ANALYSIS
The data obtained will be used to help further our understanding of sleep habits among college students in Yakima County.

RISKS
We anticipate no risks involved with this survey format and modality. To minimize any unforeseeable risks we encourage participants to conduct this survey in a private setting away from other individuals.

BENEFITS
All data points collected will help us better understand Yakima County’s college population and its sleep habits. No rewards or gifts will be exchanged or given for participation.
