When you have a full time job and SP and add a prn job on the side, do you submit paperwork to the state for a second SP?  I remember when I got my license I had to give them my SP's name and now my license is attached to his.

Also, my malpractice is covered through my main job.  Is it customary for a prn job to be willing to cover my malpractice or should I plan to get my own plan?

Yes you have to list all your SP that you use with the state you are practicing in.  You need to ask your PT/per diem position if they are offering malpractice insurance.  If not obviously you need to get your own policy which I am sure you know can be quite expensive.

Each state is different.  For example, in Ohio, you have to have a supervisory agreement but it does not have to be submitted to the state.  Both you and the practice have to keep a copy.  In KY, you have a designated primary and alternate supervising physician, and the state has to be notified of each.
