Yes. I think it's fine to just put as much information as you have. I worked a CNA job with super high turnover about a year ago, and I hardly even knew who my supervisor was (and she quit after a month). Needless to say, I didn't have her email or last name, and she does not even work for the nursing home anymore nor would she remember me. So I just put the physical address and phone number of the business. I think a school would only call or email to verify your hours. So as long as they can figure out some way to do that, you're good.
