any tips/tricks for cleaning dirty lacerations prior to suturing?

Recommended Posts

Hey gang, do you have any tips / tricks for lacerations that have tons of dirt particles completely adherent to the wound bed?  I've had a few cases where we irrigated a ton with high pressure, tried running soap and water under sink for minutes, tried scrubbing, even manually picking out the micro dirt adherent specs, but we were still left with dozens of tiny little particles left and therefore couldn't suture the wounds primarily.  I asked my colleagues but they didn't have any other ideas.  I'm sure you all have encountered this... does anything seem to work?  Would completely immersing it / soaking it for an extended period of time help?  Maybe hydrogen peroxide?  Maybe just deride that tissue out?  Thoughts?
