Practicing primary care in a big city like here comes with much limitation, which sucks sometimes Yes, I am rural, which is nice because I do have the opportunity to do a lot more, BUT it also comes with its headaches. The lack of specialists available - or the distance patients have to drive to get to one - is frustrating and a barrier to care and unfortunately there are just things I don't feel comfortable or absolutely CANNOT do. The good news is that I have built a reputation of not requesting "stat" or "urgent" referral visits without meaning it. The specialists I refer to regularly know that when I say "stat" or "urgent" I MEAN IT!!! Rural medicine also comes with a much higher concentration of controlled substance patients - which I am actively trying to work on and having to be very focused on ignoring the impact it has on my income (a large portion of my income is RVU production bonus).
