Advice for new GI PA please!

I have been made a "soft" offer for a GI position without formally discussing salary. I have been out of school and in primary care just two years. This is a busy clinic where I will be seeing approx 40 patients per day while the doc mostly performs all of the scopes on site. It's pretty much "bankers hours" without any weekend call. Two days of the week will be spent at a sattelite clinic 30 miles away from the primary clinic. The clinic accepts all insurances with the exception of medical (medicaid). The practice is in the central valley in California. So my question is what do you all think is a fair starting salary? Whats "standard" for vacation/CME/retirment? Any input would be GREATLY appreciated.

40 patients per day is too much - 30 is a really busy schedule and it shows you SP is going to make a killing off you

 

 

in general

Vacation 4 weeks

sick personal 1-2 weeks

holidays 5-9 (this will be set by the office schedule)

CME 1000 is LOW 2500 is great

full health family health included is great - but one job I was offered they paid 50% of individual meaning I had to pay almost $1k/month for family... no way as every other job except for one I had full family insur provided for me

should have around 100 patients per week - more then that and you are just a cash machine...... okay with doing it as long as you get a bonus for collections over 2x's your salary

 

heck make the pitch for 40% of your collections - collect $350k - get 120+

 

 

Personally I think we all should be on productivity in the 35-40% range as I see the local doc's just trying to use PA's as profit centers and not as a coworker/partner in their practice to deliver better care...

I am a PA who works in GI in New Jersey. Salaries vary by location. I agree with much of what ventana says. However, I am salaried, so it does not matter how many patients a day we see. A production based salary is probably the most fair, however, it is difficult to track that especially because some insurances do not pay for PAs services or require you bill under the physician's name. Be very wary of a strictly production based salary. You should ask for a base salary PLUS a yearly bonus based on productivity. Benefits are at your discretion based on what you need. I do not get any medical benefits at all. I get $750 in CME, full malpractice insurance paid, and a small pension which I earned after I have worked for 2 years. You have to weigh what you need versus what you can do without. For example, I work 15 minutes from home and do not have any night or weekend work whatsoever. That is enough to keep me happy in exchange for more CME money or more salary. I'm not afraid to say that I've been a PA for 3 years, two of which have been in GI, and I make 71,500/yr. Have been wondering myself whether this is market rate or if I am underpaid...

40, or even 30 per day in GI is crazy, unless the situation is a very eclectic one where you don't get very involved with the patient. The most I've ever seen in a day in the outpatient setting is like 19 or 20, and that was bad enough. 14 is about my average. Inpatient is a different matter altogether, but outpatient wise, where all the time and academic handling of the cases occur, you need time. I see 20 minute follow ups, 1 hour consults, and 40 minute hospital discharges and new-to-me patients. That said, we all manage our patients longitudinally, and they don't see a doc unless they, or we, request it. It is generous, but 2/8 docs and 1/6 of the other NPPs sees 30 minute follow ups. Plus, you pile on a whole bunch of 20" follow ups in a row without an hour consult to break them up, you're behind in a hurry. GI is notorious for patients with million dollar workups who don't get better.

I'm not afraid to say that I've been a PA for 3 years, two of which have been in GI, and I make 71,500/yr. Have been wondering myself whether this is market rate or if I am underpaid...

 

 

wow you are seriously underpaid - the health insurance alone is worth 5-15k per year

most recent data from AAPA shows 1-4 yr PA's in Jersey getting about 87-90k per year with full bennies.....

oh BTW you are making less then the 25% for a new grad PA!

 

This is why I always think it is wise to get the AAPA most recent data before signing on to a new job - you are worth more then you think!!

I recently had interview with a G.I. specialist

He is also requesting:

 

1) Me seeing about 4-5 patients/ hour (some docs are claiming this is the average) per day= about 40 patients per day!

(Is this in any way reasonable?)

 

2)He wants me to get a new cell phone so he can text me and communicate real fast.

 

3)I have about 20 years experience in internal medicine and have been in many areas..

 

4)He may pay me 100k/yr..So far benefits are medical ,questionable dental, definitely malpractice..

 

5)There could be on call one time per month

 

6)20% of my time could be (in patient) G.I. medicine

 

7)Lastly the meticulous Hep c treatments was thrown in.

 

8)His offices are only 20 minutes from my house, no toll..

 

Any feed back is appreciated! [since I did'nt go to school to negotiate]

Make him pay for the cell phone!!! I learned that the hard way... Also I don't see how you can see 40 patients a day in the office AND inpatients. And what does "On call one time per month" mean? Is that a week, a day, or a weekend? Also you said his "offices"... does that mean you will be travelling between multiple offices AND the hospital? You didn't mention CME either... that should definitely be included.

I have to work from 2 different offices, one of which is 16 miles away from home. I have a soft offer from another group where they are expecting me to round at 2 hospitals but the location of the office and the hospitals will be closer to home (approx 15 mins). I will also have to take weekend calls. I'm wondering how much I should ask the new group.

Great advice so far.  My current scenario I see 20-25 patients per day on a typical day, but I come in at 9:30.  If I was there at 8 and left at 5 I definitely could be in the 30-35 range.  I have had 30-35 patients in a day and it was tremendously stressful.  You never know what will come in every day.  Some are very complicated, don't have complete records or need an admission.  40 patients is too much.  I would say try to stress that you would like to cap it at 30.  Even at average of 20 per day my MD is collecting twice what my salary is so they are going to make money off of you.  My last interview was 93k base and then 50% of collections after my expenses/salary are taken out.  I am waiting for a formal offer as I have asked for a part time schedule and not sure if it will work for them.  This is about the same as I make now I just make a higher base rate with lower bonus at my current position.  They said most of the full time, productive PAs make 120-130k.

It amazes me how much pay can vary from specialty and state.

8 hours of pt. interaction per day where you are doing nothing but moving from room to room at a rate of 30 patients per day averages out to 16 minutes per pt. for rooming pt., the encounter, and documentation.  I know nothing about yourself or your setting but in a specialty clinic 16 minutes per patient doesn't seem to allow much time for information gathering either by history, exam, or both.  I remember back in the early 80's when we were in many ways glorified MAs, my doc (specialty surgeon) would book follow up visits on 15" time slots and to keep up he was dictating in the room with a hand recorder (pt's hated it) and we would still run behind.
