rcdavis Posted July 22, 2011 Most the time when I give narcan, as soon as the PT opens his mouth, I immediately regret it.

Contrarian Posted July 22, 2011 ... narcan 2 mg intranasal... Hey E... Where did you guys snag the atomizers...??? (company & NSN-Stock number please) I've been trying to convince folks at one of my jobs to switch to intranasal Benzos for our agitated combative patients... and would like to have this option at another job for detox. Thanks Contrarian

ohiovolffemtp Posted July 22, 2011 Once upon a time, one of my co-workers gave 2mg narcan to an OD. When he saw the pt. start to move, he put his hand on the pt's forehead and said, "Demon, I command you to come out of this man!" - the pt. woke up, and the EMT-B on the run freaked out. This run became part of the folk-lore at my dept.

flyingsquirrel Posted July 22, 2011 We've been using these at my EMS job for a couple of years and I love them. We get ours from Bound Tree Medical, item #400124 (with 3cc syringe) or #400125 (without syringe). I don't know the NSN, sorry. The website for the manufacturer is http://64.245.154.166/MAD/MADNasal.aspx

medic25 Posted July 24, 2011 When I was a newish EMT, I was partnered with a very burnt-out medic who had the craziest narcan wakeup I've ever heard of. He responded to a respiratory arrest/heroin OD. After intubating the patient, he started an IV and pushed narcan, promptly waking the patient up. Instead of extubating the patient, he kept the tube in and transported. When they got to the ED, he had the patient stand up and stick out his index finger. He then hung the IV bag on the patients finger and walked him, ET tube still in place, into triage!!

medic25 Posted July 24, 2011 +1 for the MAD. We've used it in the field for a couple of years to administer both narcan and midazolam, and have found it to be very effective. With the next update to our protocols we are looking to add IN fentanyl, primarily for pediatric analgesia.
