Physician (Family medicine) who I work for part time in his medical office offering me to visit his patients in Nursing home /Rehabilitation center for $20/patient/day. 20 patients per week.

Is it a standard pay?

I would say there is minimal overhead, and you should do it for no less then 50% of collections.  Simple

Do you know any HCPCS Codes for the Nursing home /Rehabilitation center? I will not perform any procedures there, just visit.

The amount he's offering to pay you is ridiculous...You should expect between $30-80 - $30 for a follow up and $80 for an initial visit.  Be careful, he sounds scrupulous as most SNF/LTC work is extremely lucrative.  He's most likely trying to keep the majority of the profit for himself.
