  1. Hello so first off, I'm a senior in high school interested in the health field, and looking mainly at the physician assistant profession. Currently, I don't seem to have the drive to go through 4 years of med school + and many more years of residency. I would also hate to pay off all the debt as well. I want to finish school early, less debt, start a family, and then possibly start a business. However, I heard that PA's have a hard time starting their own clinic and they must have a MD with them all times, is that true? The main reason to start my own clinic is because I want to be a little more independent from a MD, and I like business also. Which states allow me to practice independently? I'm from New York. Now how will I make money? Will I have to pay the doctor more money than me, if my business becomes successful?

  3. Hello! I am a first year PA student and I am wondering if anyone knows of any free case study resources that are available. I am looking for a way to practice integrating and applying classroom knowledge to cases. If it would be worthwhile, I am willing to buy case study books, but would like to first find some websites if possible (being on a college student budget, of course!). Thanks in advance!
  4. I started my practice in an ED at a level one trauma center and was informally trained in bedside ultrasound on the job. Over my 3.5 years with the group, PAs were brought into the same rigorous training standards as the EM residents with the goal of securing credentialing for all PAs. I ultimately completed the requisite exams and was technically credentialed at that point. I left that position shortly after and began working with another area organization in EM. Bedside ultrasound is culturally used less as the average practitioner with my current group has less experience, however many of the staff are ultrasound trained. I'm running into issues with ultrasound documentation in that we are allowed to perform the exam, bill and document only if we have a credentialed physician looking over our shoulder while we perform the exam. We have been asked not to document the ultrasound in our note for both billing and medicolegal reasons (understandably). When this issue for APPs (both PAs and NPs in my new practice) came up, our group was told that it is system-wide policy that APPs can't do or can't be credentialed to do bedside ultrasound. I'm wondering what resources are available to help PAs/APPs develop a credentialing process for bedside ultrasounds within their organization. I see there is a Society of PAs in Clinical Ultrasound, however not many resources regarding this professional practice issue. Can anyone help point me in a direction or offer up their professional experience?

  7. Getting some advice from my state APA that legally it sounds more kosher to refer to my House Calls business as a "Service" rather than "practice". Any thoughts about this? I am hatching a business: providing a " service to Doctors" So, marketing to them. Huge interest so far whom ever i approach. =
  8. Minnesota is a great state in which to practice as a PA. The medical culture here is fairly PA-positive, as well. I'm very happy with my full prescribing rights, my off-site supervising, the two meetings in person per year with my collaborating MD. But oh man, is it becoming a good environment for the NPs. A recent change to state law removed the need for NPs to have oversight from MDs, and the U of MN is actually opening a neighborhood primary care clinic staffed entirely with NPs. http://bringmethenews.com/2015/03/25/the-nurse-practitioner-will-see-you-now-u-of-m-set-to-open-nurse-only-clinic/ http://www.mprnews.org/story/2015/03/24/nurse-clinic http://www.mndaily.com/news/metro-state/2015/03/24/u-opening-first-nurse-led-clinic Personally, I have mixed feelings. I wish them all the success in the world, but at the same time I really hope someone other than just the NP Program at the U is going to be tracking their data, especially as it pertains to outcomes for their patients. This looks to be an unusually ripe opportunity to measure the quality of NP care where the rubber meets the road. I also have worries about the trainee NPs who could conceivably come up through their educational process with minimal interaction with MDs and DOs, much less PAs. I wonder if some of those oddball ingrained beliefs about us will carry farther through training, and into practice? Not to mention, anything that smacks of "separate but equal" kind of raises my hackles. I can't think of a time in history that's ever been good for anybody. This could be a raging success, and in the best of scenarios it could potentially be a wake-up call for Minnesota's docs, strengthening the link with us PAs as their full partners. It could even lead to further expansion of our practice act, and more independence. On the other hand, it could be just enough rope for the NP profession to hang itself. It's gonna be an interesting few years, watching this story.
