  2. Yes those are the two forms. If you have more than 1 job related to healthcare/patient care make a copy of the form and have the supervisor fill it out. Once they are conpleted by the supervisor and PA, you upload them to CASPA. Sent from my SM-G950U using Tapatalk

  6. I am not sure. You should reach out to the program and ask them. I know most PA programs prefer classes that have been taken in college but, they may be lenient if it is a non-science class.
