Contract offered. Criticism/opinions welcome.

Hi there,

 

I will be moving from Harrisburg, PA to Dallas, TX in the next few weeks. I've worked 3 years in family medicine and was offered an urgent care job in northern Dallas/Richardson recently.  Compensation is $60/hr with monthly volume based incentive bonus (unsure of how much...), full benefits, 3% 401k match, 15 days time off, 1000 cme, 3 days for CME. Sounds like a decent deal to me...but I don't want to jump on something too quick. Any thoughts?
