New Grad FM vs UC (both good offers!!)

First of all, thank you to everyone on this forum for offering your wisdom and guidance. So grateful to have you guys to turn to! Would love help with one more.

 

Both positions are in Southern CA (but are in an area w/ much lower COL than most cities in SoCal)

 

OPTION 1

Family Practice for large medical group

 

115k salary. no call, no nights, no weekends. 40 hrs/week M-F 8-5

-100% Medical, dental, vision covered for employee + dependents

-240 hours per year of PTO (includes sick time and CME)

-CME Stipend: $1500

-Claims-made malpractice insurance covered but NO TAIL COVERAGE

-License and Fees covered, including DEA and Boards

-Flexible Spending Account (FSA) for Health Care (up to $2550) and Day Care (up to $5000)

-Life Insurance: $50,000

-Employee Assistance Program (EAP)

-Jury duty-one week paid

-Bereavement leave: 3 days paid per death

-Workers' Compensation

-Long-Term Disability: 60% of income

-Retirement: 4-6% of salary contributed to retirement by the employer regardless of employee contribution in addition to a 50% match up to 6% (Starts after one year of employment)

-Non-compete clause....must get permission from employer before working anywhere else as a PA

 

OPTION 2

Urgent Care for an academic hospital - CT, IVs, ultrasound, xray all available on site (almost like a mini-ER). To give you an idea, they have beds and a fast-track area in this UC

 

-$55/hr + $5K SIGN ON BONUS, 12-13 hr shifts with average of 12-13 shifts per month (they said the minimum is 72 shifts in 6 months)

-13% of salary contributed by the employer into 403B; no match required but able to voluntarily contribute your own % [starts right away]

-10 paid sick days

-NO PTO :(

-Disability cap: short term $325,000, Long-term $400,000

-Life insurance: $300,000

-Health plan for employee + dependents w/ medical, dental, and vision [$40/month individual or $260/month family-don't have to worry about that yet plus my spouse has great insurance benes]

-4 days jury duty

-flexible spending account

-Malpractice fully covered: Occurrence policy (so no tail required! yay)

-Employer paid licenses, fees, dea, boards, credentialing etc

-no non-compete clause; work wherever unrestricted plus having more days off means it's easier for me to moonlight elsewhere

-$4000 CME!

 

Clearly both are pretty good offers for a new grad but they also have their own weak areas. I like both practices and feel as though both would be great learning environments.

Both are good offers. I'm in UC now and some advice I would get clarified first would be.

1) how much training time will you get

2) are you a solo provider. I'm all alone and it's been hard. My SP is available by phone and responds very quickly but it's taken some getting used to. So just know where your resources will be.

3) is radiology over reading X-rays? Are you required to read CTs?

The CME is great there. Salary seems pretty good for new grad. Also really good for IM practice. But again I'd clarify how much training you'll get. How many pts your expected to see in each place and where is your SP.

 

You'll get great experience at either place. Make sure you're comfortable and feel good about the place you decide. Good luck! Congrats getting two offers in so cal. I was in San Diego for months with very little job openings to choose from!!!!

  • For the Urgent Care. They said there is a three month training period for the “fast track” area followed by three months in the “main” UC working up the belly and chest pains, etc.
  • Would not be a solo provider at either site. At the UC, there are always 3-4 providers every day. 1-2 attendings/SPs and 2 NPs/PAs. One of the things I love about this place is that it IS a unique UC, not your typical strip mall UC. At the FM group, there are 2 FP docs, 1 IM doc, and 1 ob/gyn doc. Each have a PA/NP that works with them. I would be working with one of the FP docs they brought on board this year but can get help from any of the other providers.
  • Imaging would be read by the radiology department in the medical center across the street but I also enjoy looking over my own films for learning purposes. Did a small elective rotation in Radiology
Patient wise I would be expected to see about 20-24 patients per 8-hr day at the FM. They would expect me to get to this point at ~6months. Urgent Care-after my training period pts every 15 minutes in fast track and 2pts/hr in the main UC. They hired a new grad from the class before me and are familiar with the learning curve required

 

Thanks for your feedback!! I’m quite sure offers like this don’t exist for new grads in San Diego (trust me I’ve looked) so I had to look at nearby counties instead

You're going to want tail coverage for family med. You have a pretty good chance of getting sued in that specialty if you stay there long enough.

 

Also, with the non-compete clause, you wrote: "Must get permission to work anywhere as a PA". Does that mean anywhere as in literally anywhere in the world? I wouldn't sign that, though I can't imagine how that can be enforced.

 

Other than that, these are solid offers.

I read that as a no-moonlighting clause, because it's far too vague otherwise and nonsensical for it to extend beyond the duration of the employment.

i personally would take option 2. 13% contributed to your retirement is unheard of.

Even though no PTO, is there opportunity to pick up extra shifts, or would they be flexible and allow you to front load your shifts in a week and backload on the next week to allow u to take weeks off if you wanted?

Thanks PAtoB. Yes you can pick up extra shifts - full shifts or half shifts but no OT pay which is why they don't care if you work more. Able to request 10 days off/month so they can front or backload shifts around those 10 days. Also if you work say 14 shifts in January you can say just schedule me for 11 shifts in February. So really flexible.

 

Time off is scheduled on first come first serve basis and each provider is required to work 3 weekend days/month so they won't stick the new grad with all the weekend shifts which is really nice :)
