unclear on the concept....chf management....

55 yr old male seen recently with CHF exacerbation with edema to the level of the scrotum(think softball size)....Lasix "doesn't work" so he decided to take matters into his own hands....by lacerating his scrotum at home with a pocket knife to allow it to drain....needless to say, a bad idea.....

reminds me of a psych call way back in college on EMS

 

Must have waited outside for a solid 30 min with the police secured the scene.  (no way I was going in with out that)

 

walked in to find a very pleasant thin male in his 50's literally laying in a pool of blood, I was 3rd, the Crewchief started to turn blue....  When the answer to the "Where did you cut yourself" came they literally turn and fast walk/ran out the door.  Leaving me alone (probably for the better at that point)

 

Turns out this person was a psychiatric patient that had signed out AMA (sad) and decided to get rid of testicles.  Then was bleeding so much sewed himself back up with a sewing needle and thread, then waited a day to call us.  So very sad that the system failed so miserably.
