My employer wants me to get the ball rolling on the paper work for licensing so that I can start as soon as possible and I have absolute zero clue on how to fill this stuff out. Does anyone have any idea how this stuff is to be filled out? What parts I need to fill out? What parts I need to omit? Any help would be greatly appreciated.
