For those of us who work in the developing world at times they get a taste for this. Speculate: What would health care in America look like if there was no such thing as lawsuits or insurance companies (but all care was free)? I would tell pts the truth more often (there is nothing medically wrong with you. you need to be on a psych med...)and order 1/2 as many tests. my charts would all be on 3x5 cards like they are in Haiti: c/o X, exam of Y. considered Z. they don't have it. Dx A, tx B treatment would consist of what pts really need without consideration of what they want and will call admin about if they don't get....yes, I am guilty of the zpak for bronchitis just to get cranky pts to leave when I am busy... We're all guilty of it and we still teach our students not to do it. I agree with what you've said 100%. I would order fewer tests. I would use fewer meds.
